Healthier Future(“we”, “us” or “our”) provides this patient portal to help you manage your care. The portal is operated on our behalf using technology powered by Phensys. By accessing or using the portal, you agree to these Terms & Conditions. If you do not agree, please do not use the portal.
The healthcare services you access or arrange through this portal are provided by Healthier Future. Phensys provides the underlying software platform only and does not provide healthcare services, clinical advice or treatment. Healthier Future is responsible for the care you receive.
Access to the portal requires an account and secure login credentials. You are responsible for keeping your login details confidential and for all activity that takes place under your account. Please notify Healthier Future promptly if you believe your account has been accessed without your authorisation. We may suspend or withdraw access where we reasonably believe your credentials have been compromised or the portal is being misused.
The portal may allow you to view, request, book, reschedule or cancel appointments. Bookings are subject to availability and to Healthier Future’s policies, including any cancellation or non-attendance policies. Confirmation of an appointment is only valid once it appears as confirmed within the portal or is confirmed to you directly by Healthier Future.
You agree to use the portal only for lawful purposes and only in connection with your own care or the care of a person for whom you are authorised to act. You must not attempt to gain unauthorised access to any part of the portal, interfere with its operation, upload malicious code, or submit information that is false, misleading or unlawful.
The portal is not monitored continuously and must not be used for medical emergencies. If you or someone else is experiencing a medical emergency, call the emergency services immediately (dial 999 in the UK) or go to your nearest emergency department. For urgent but non-emergency concerns, contact Healthier Future directly or, in the UK, call NHS 111.
Use of the portal does not, by itself, create a doctor–patient relationship or any other clinical relationship. Any such relationship arises only through the healthcare services provided directly by Healthier Future’s clinicians. Information available in the portal is not a substitute for professional clinical advice, diagnosis or treatment.
All content, software, design and materials made available through the portal are owned by Healthier Future, Phensys or their respective licensors and are protected by intellectual property laws. You are granted a limited, non-exclusive, non-transferable right to use the portal for its intended purpose. You may not copy, modify, distribute or create derivative works from any part of the portal except as expressly permitted.
To the fullest extent permitted by law, the portal is provided on an “as available” basis and we do not guarantee that it will be uninterrupted or error-free. Nothing in these Terms limits or excludes any liability that cannot lawfully be limited or excluded, including liability for death or personal injury caused by negligence, or for fraud.
We may update these Terms & Conditions from time to time. Any changes will be posted on this page with a revised “Last updated” date. Your continued use of the portal after changes are posted constitutes acceptance of the updated Terms.
These Terms & Conditions are governed by the laws of England and Wales, and any disputes will be subject to the exclusive jurisdiction of the courts of England and Wales, unless a different jurisdiction applies to you under mandatory local law.
